JEFFERSON CITY, Mo. (KMIZ)

UPDATE: Jefferson City police found Jack Hazen safe and canceled the alert. Details of where he was found and how were not provided.

ORIGINAL: The Jefferson City Police Department has issued an alert for a missing 66-year-old man.

Jack J. Hazen was last seen at Capital Region Medical Center on May 26. Hazen is about 5 feet 8 inches tall and weighs 165 pounds with brown eyes, according to the police advisory.

JCPD says Hazen likely has dementia and has alcohol addiction and traumatic brain injury.

The alert asks that anyone with information dial 911, local law enforcement or the Jefferson City Police Department at 573-634-6351.
