Records show missing reports for Second Chance Homes

The contracted case management agency for the Missouri Department of Mental Health, Callaway County Special Services was required to perform face-to-face visits with patients, including DeBrodie, every month and document those encounters.

The CCSS Action Plan Tracking System is a record of non-compliance that is reported to the state.

The system records for the Second Chance facility show more than 250 instances of non-compliance with DMH.
