Missing, inconsistent records at Second Chance facility

Documents from the case management agency for the Second Chance facility in Fulton show inconsistency with the death of Carl Debrodie.

The Missouri Department of Mental Health mandates Callaway County Special Services to perform monthly, face-to-face visits with the service consumers, such as Debrodie, and to record whether the visits failed or succeeded.

ABC 17 News obtained the records of all of those monthly visits and none of them indicate that there was a missed meeting due to the absence of a client.

Investigators say they believe DeBrodie was missing from the facility long before his absence was reported.

Callaway County Special Services announced Thursday that one of its case managers was dismissed after an internal investigation into the Second Chance Home.
